1. Eligibility Requirements:

* Children entering Pre-Kindergarten must be 4 years old on or before September 30.

* Children entering Kindergarten must be 5 years old on or before September 30.

2. Documents Needed: 

* Completed application

*Copy of Child’s Birth Certificate

* Copy of Child’s Baptismal Certificate

* Updated State of Tennessee Department of Health School Immunization Certificate 

* Copy of previous year’s final report card

* Copy of Child’s most recent standardized test scores

* Contributing Catholic Verification Form 

* Signed Transcript Release Form. Upon acceptance to ICS, the Admissions Office will submit this form to your current school to receive all permanent records.

* Non refundable application fee of $50.

4. Entrance Testing/Interviews:

* Assessments will be scheduled for students entering Pre-Kindergarten and Kindergarten. Transcripts will be reviewed for students in grades 1-8.

* Upon receipt of all required documents and assessment results (if applicable), an appointment will be set up for an interview with the principal.

5. Parents will be notified of acceptance:

Available openings for admission are prioritized based on the following:

* Siblings of registered and supporting Incarnation parishioners

* Children of registered and supporting Incarnation parishioners

* Children of new Incarnation parishioner that participated fully in their previous parish

* Siblings of non-Incarnation parish families

* Children of non-Incarnation parish families

* Non-Catholic families
